Expert Housing Stabilization Services
We provide comprehensive support to help individuals with disabilities, mental illness, substance use disorders, and seniors find and maintain stable, long-term housing. Our team assists with navigating challenges like budgeting, lease agreements, and building positive relationships with landlords, while also connecting clients with essential resources for mental health support, financial assistance, and other community services. By addressing both housing and outside issues, we ensure our clients achieve lasting stability and thrive in their homes and communities.
To qualify for Housing Stabilization Services (HSS) in Minnesota, clients must meet the following criteria:
- Age: Must be 18 years or older.
- Enrollment in Medical Assistance (MA): Clients must be enrolled in Minnesota’s Medical Assistance program.
- Experience of Housing Instability: Clients need to demonstrate one or more of the following housing challenges:
- Homelessness
- Risk of Homelessness
- Risk of Institutionalization
- Transitioning from an institution or licensed/registered setting
